Reminder: Dependent Care Flexible Spending Account

Please remind employees who participated in the Dependent Care Flexible Spending Account for the 2013 plan year that the last day to submit claims incurred during the 2013 plan year (January 2013 - December 2013) will be March 31, 2014.

Claims received after that date will not be reimbursed, and federal law requires that any money left in the employees' 2013 accounts be forfeited.  Claims should be submitted to ADP using the attached form.  Additional questions should be referred to ADP via the Kindred HUB at 800-991-6171. 

Dependent Eligibility Audit Deadline

The Dependent Eligibility Audit Deadline is March 28!  Please remind your employees! 

Our dependent eligibility audit is now underway.  While many employees have already responded, we have others who still need to respond.  As a reminder, the audit began March 10, 2014, and ends on March 28, 2014. During this period, all employees who have dependents actively enrolled in the medical, dental, or vision plan are required to update their dependent information and provide documentation as proof of eligibility. However, any dependent that was added and verified in 2013 or 2014 will not have to be revalidated.

W2's

2013 W-2's will be available for Pick-up on Thursday January 23, 2014

Employees: W-2's may be picked up at the Security Desk (Lobby) at Phoenix and the Reception Desk (Lobby) at Northwest during Lobby Hours (M-F) beginning tomorrow, Thursday (1/23/14).  All W-2's must be picked up by 3:00 PM, Thursday, January 30th.  W-2's that are not claimed by the deadline will be mailed. You must present an ID (Kindred Badge will be sufficient for current Employee’s) for pick-up.

Tobacco Free Reminder

Dear Staff,

Thank you for all of your assistance in making Kindred Hospitals Phoenix and Northwest Phoenix a Tobacco – Free Workplace.  It was a major change for some of our staff, visitors and even some of our patients.  Kindred is committed to providing a healthy and safe environment for our employees and others who enter our company premises and recognize that tobacco and tobacco smoke pose a significant health threat to anyone exposed to them.

Unfortunately it has been noted that some employees are not following the policy.

Reminder:

• There is no designated smoking area on company premises for employees, patients or visitors.
• Non-exempt employees may not leave hospital premises while they are clocked into the time-clock system (on duty) such as when they are taking a 10-15 minute rest break.
• All non-exempt employees wishing to use tobacco may only do so by clocking out of the time-clock system (off duty) such as when they are taking meal breaks.
• All employees (exempt or non-exempt) must leave the facility premises to use tobacco products.
• Employees who continue to violate this policy will be subject to the progressive discipline policy as stated in the Employee Handbook.

Traveler Update Reminder

Kindred Travelers,

Just a friendly reminder that the Kindred Travel Department will close Friday, October 25, 3:00pm ET in preparation for the travel transition events previously communicated.  Concur Travel will also be unavailable at this time.  Travel services and Concur will be available Monday, October 28, 8:30am ET.  Concur Expense users will still have access to the system during the transition period to submit expense reports.

In the event that you require emergency assistance with your travel during the transition period, please contact the ALTOUR support line for assistance.

American Lung Association Walk

Thank you to all who have signed up to join us Saturday April 27^th for the 2013 Fight for Air Walk.  If you haven’t yet registered, you can do so on the ALA Website under team name “Kindred Krusader’s”, or simply by signing the sign up sheet located at the front desk of each facility.  The registration fee is $10 which includes a pancake breakfast, a team spirit t-shirt to be worn the day of the event and a morning full of good times J  Please help us help others by encouraging friends, family and staff to join us in our “Fight for Air”.

**t-shirts will be provided by Kindred to those who have paid their $10 registration fee by Thursday, April 4^th.  T-Shirt order must be placed by Friday, April 5^th to ensure shipment by the April 27^th Walk date.  Please email me your t-shirt size ASAP!

W-2's may be picked up at the Reception Desk between 10:00 AM – 8:30 PM Mon- Fri and 11am-7pm Weekends, outside these hours the please see the Nursing Supervisor.  All W-2's must be picked up by 12 PM, Tuesday, January 29th.  W-2's that are not claimed by the deadline will be mailed. You must present an ID (Kindred Badge will work) for pick-up.

Please keep in mind that if not picked-up; the address on your last paycheck of 2012 is where your W-2 will be mailed.

Please see Payroll or HR if you need to submit a change of address.

Employees may only pick up “their own” W-2.

Healthy Steps: Know Your Numbers

Know Your Numbers – Both you and your covered spouse/domestic partner must complete a health screening from January 1, 2013-March 29, 2013 in order for you to maintain the Healthy Rewards rate through September 30, 2013.  Beginning January 1, 2013 go to www.healthystepswellness.com and schedule your appointment at a local Quest Diagnostics Patient Service Center or if you prefer obtain a form and visit your Primary Care Physician.

KEEP IN MIND:

Challenge Yourself – Both you and your covered spouse/domestic partner must participate in special healthy challenges throughout the year and earn 4000 points by September 15, 2013 in order for you to maintain the Healthy Rewards rate through December 31, 2013.
