The
Dependent Eligibility Audit Deadline is March 28! Please remind your
employees!
Our
dependent eligibility audit is now underway. While many employees have already
responded, we have others who still need to respond. As a reminder, the audit
began March 10, 2014, and ends on March 28, 2014. During this period, all
employees who have dependents actively enrolled in the medical, dental, or
vision plan are required to update their dependent information and provide
documentation as proof of eligibility. However, any dependent that was added and
verified in 2013 or 2014 will not have to be revalidated.
A
letter was mailed to the home of each impacted employee on February
28th listing all of their covered dependents, noting those who
required verification. This letter also gave instructions for how to submit
their information.
Employees can submit
their dependent documents by uploading them to the website, by fax, or by email.
Please remind them to include the attached Dependent Audit Fax/ Email Cover
Sheet when sending their documents via fax or email. The attached flyer details
the documents that employees will need to provide.

At
the conclusion of the audit, any dependent for whom we do not receive proper
documentation will be removed from the medical, dental, and/or vision benefits
effective May 1, 2014.
